Interventions

When the risk stratification process is completed, targeted interventions are tailored to the needs of the employer and each individual.

Strategic Health Services offers programs that promote long-term lifestyle change and health improvement, including:

Lifestyle / Preventive Care Programs that address issues that account for about 80% of all health related costs. Strategic Health Services has health coaching that provides behavioral support and education, offered via the Internet, telephone and/or mail for:

  • Smoking cessation

  • Stress management

  • Weight management

  • Exercise

  • Nutrition

  • Pre-Diabetes

Chronic Condition Programs delivered through one-on-one interactions and disease management programs with experienced registered nurses, therapists, and other health care professionals. Conditions addressed include:

  • Hypertension

  • Coronary artery disease

  • Congestive heart failure

  • Hyperlipidemia(High Cholesterol)

  • Asthma

  • COPD

Case Management / Health Advocacy Programs support employees with a team of diverse and experienced healthcare professionals who provide assistance with a wide range of health-related issues.

Where appropriate, Strategic Health Services staff collaborate with physicians to educate and ensure compliance with medical protocols, treatment plans, and prescriptions.
