To determine what's driving up your health care costs and what kind of return on investment you may get by investing in health management, follow the instructions below:

1.	Obtain your claims data, grouped by claim codes as shown on the calculator.
2.	Fill in the answers to each question, including your total annual health care expenses for your group health plan, as well as by claims codes. (Be sure to include all medical claims for chronic diseases from all points of service.)
