Health Risk Assessment

The confidential online Health Risk Assessment coupled with biometric screening options captures health and lifestyle data and is the basis for a personalized health profile that provides a holistic view of each employee's health status, specific health risks and health score. This questionnaire also serves as a baseline against which participants' progress and future health risks may be measured.

Biometric Data

In addition, electronic brochures on chronic conditions and lifestyle issues are also available.

Provides for ongoing comparisons of progress and results with storing capabilities of previous year's lab results, Personal Health Profiles, and Health Risk Assessments. This allows for easy access to current health information for personal reference, a place to document progress and relevant information to share with personal physicians and also for year-to-year comparison of health risks and progress.

Through the My Pathway to Health website, secure messages can be conveniently sent to and from the employee's personal Health Coach to provide health updates and discuss health issues or questions.
